Enrollment Agreement and Legal Contract

General Provisions:

This Agreement Must Be Signed 6 Days Before Beginning of Class.

I understand that I will be entitled to the privileges and bound by the conditions below:

Cancellation: I understand that I may cancel this Enrollment Agreement in writing, within six business days of signing the agreement, and will be entitled to a full refund of any money paid.

Financial Obligations: I understand that I am personally responsible for all tuition, fees and other charges arising from and during my enrollment at Midwest Training Institute of Hypnosis.  

I understand that it is my personal obligation to pay all tuition, fees, and other charges when due. If I do not pay the full amount of any scheduled payment when that payment is due, I may not be permitted to continue my studies. If I am not able to continue my studies, I understand that I am responsible for any outstanding tuition, fees, or other charges due in accordance with the Institute’s refund policy. I understand that my academic transcript   will not be released to me or to any other individual requesting my transcript if there is a balance due to Midwest Training Institute of Hypnosis. In addition, if I choose to re-enroll at Midwest Training Institute of Hypnosis, l must satisfy any outstanding tuition, fees, or other charges prior to my re-enrollment.

Instruction: I understand that I will receive instruction in English, in lecture and laboratory formats. Instructions will be delivered by qualified instructor Midwest Training Institute of Hypnosis.

Renewal of Enrollment Agreement: I understand and agree that although this agreement is executed for a period of only one (1) class, my enrollment at Midwest Training Institute of Hypnosis for, subsequent classes shall constitute a renewal of the terms of this agreement except for the tuition charge and fees, which may be subject to change with at least one class notice to students. 

ACKNOWLEDGEMENTS:  I understand that Midwest Training Institute of Hypnosis reserves the right to make changes in program content, materials, or schedules as it deems necessary.  The Institute further reserves the right to discontinue my training for unsatisfactory progress or attendance, non-payment of tuition or fees, or failure to comply with the Institutes policies and procedures.
                 

I understand that my tuition charges are for the right to attend classes in which I am enrolled and are in no way contingent upon my satisfactory academic progress, personal satisfaction, or attainment of employment upon graduation,

I have received and read a copy of the Midwest Training Institute of Hypnosis academic catalog. I have read and understand this enrollment agreement and I acknowledge receipt of an exact copy of the same. I understand that this agreement contains all the terms of my enrollment and acknowledge that no verbal statements have been made contrary to what is contained in this agreement. My signature below certifies that I have read, understood, and agreed to my rights and responsibilities, and that the College’s cancellation and refund and that the Institutes cancellation and refund policies have been clearly explained to me.

Sec. 6.5

(a) The post-secondary proprietary educational institution shall pay a refund to the student in the amount calculated under the refund policy specified in this section or as otherwise approved by the commission. The institution must make the proper refund no later than thirty-one (31) days of the student’s request for cancellation or withdrawal.

(b) The following refund policy applies to each resident post-secondary proprietary educational institution, except as noted in section 4.5 of this rule:

  1.   A student is entitled to a full refund if one (1) or more of the following criteria are met:  The student cancels the enrollment agreement or enrollment application within six (6) business days after signing The student does not meet the postsecondary proprietary educational institution’s minimum admission requirements. The student’s enrollment was procured because of a misrepresentation in the written materials utilized by the postsecondary proprietary educational institution. 
  2. A student withdrawing from an instructional program, after starting the instructional program at a post-secondary proprietary institution and attending one (1) week or less, is entitled to a refund of ninety percent (90%) of the cost of the financial obligation, less an application/enrollment fee of ten percent (10%) of the total tuition.
  3. A student withdrawing from an instructional program, after attending more than one (1) week but equal to or less than twenty-five percent (25%) of the percent (75%) of the cost of the financial obligation, less an application/enrollment fee of ten percent (10%) of the total tuition.
  4. A student withdrawing from an instructional program, after attending more than twenty-five percent (25%) but equal to or less than fifty percent (50%) of the duration of the instructional program is entitled to a refund of fifty percent (50%) of the cost of the financial obligation, less application/enrollment fee of ten percent (10%) of the total tuition.
  5. A student withdrawing from an instructional program, after attending more than fifty percent (50%) but equal to or less than sixty percent (60%) of the duration of the instructional program, is entitled to a refund of forty percent (40%) of the cost of the financial obligation, less an application/enrollment fee of ten percent (10%) of the total tuition.
  6. A student withdrawing from an institutional program, after attending more than sixty percent (60%) of the duration of the instructional program, is not entitled to a refund.
